Disciple Maker Index Survey
Dear Parishioners,
Our Parish is on a journey to know, love, and serve Christ more deeply as a faith community. Together, we must embrace Christ’s vision for missionary discipleship so that OLPH can thrive—a place where all can grow together in faith and be strengthened in our relationship with Him.
An important step in this process is assessing our greatest strengths and opportunities, and that is where the Disciple Maker Index (DMI) Survey comes in! Developed by the national Catholic Leadership Institute, this survey is a powerful tool that invites you to reflect on your discipleship journey here at OLPH and provide input to help our parish grow. The DMI is designed to:
- Invite parishioners to reflect on their journey of discipleship.
- Enable parish leadership to make data-driven decisions.
- Help parishioners grow in their faith.
Your input will help strengthen our parish and prepare us for the future. The survey will be available online from March 5 through April 10, 2025. You can access it at:

The survey takes 10–15 minutes and can be completed on a smartphone, tablet, or computer. It asks you to reflect on your own spiritual growth and provide feedback on the efforts of our Parish and Archdiocese to help you grow—what we are doing well and where we can improve.
All responses are confidential. OLPH will only receive summary results about our parish as a whole.
For those who prefer a paper copy, surveys will be available in the Church sacristy off the vestibule and Rectory office beginning this weekend. You may return completed surveys in the collection basket at Mass or drop them off at the Rectory by April 10.
We are striving for the highest response rate possible because your feedback is essential. This information will be valuable to Archbishop Pérez, our Parish leaders, and me as we plan for the future and strive to become the best disciples we can be. Once we receive the results, we will share what we have learned.
At OLPH, we are blessed in so many ways. Together, we will identify our strengths and opportunities for growth in greater detail as we continue to grow in missionary discipleship.
